fix for ticket #443, have added an extra column (nicked sql code from matts screen 81), note it runs slowly on the supplier only report. However there is another issue with the report in a yet to be raised ticket, there are duplicate quote references in the report, so this could be looked at together.

Fix for ticket #438 (SC1003), agents could see the create party option/button. They should not have been able to view this screen. Restricted the create party menu option and create party button so that only webmipadmin can see them.

Fix for #368 any details entered on the enquiry that were not explicitly saved were lost when the user attempted to upload a document. Altered the upload button to ensure that all changes are saved back to the database before the file is uploaded.

fix for ticket #365, the code was trying to get the supplier address changed this to the agent address, also the code was never going to get an address as the call to get the address occurred before we got the supplier/agent ids, so it was always passing a null as the supplier/agent id.
